RS-670 SCUBA REGULATOR SAFETY RECALLED BY TUSA
TUSA has announced a safety recall on its RS-670 regulator distributed or sold from May 2009 through September 2009 with retail price of $449.00.
Hazard: Loosening of the BLC plug may cause a high-pressure leak creating an unstable intermediate pressure. Immediate return of the affected units is requested.
Action: • DEALERS: Contact customers who have purchased RS-670 units and request return of their units to your store for inspection. • All owners and dealers with RS-670 units within the above listed range are requested to return them to TUSA for replacement under warranty. • Contact TUSA Customer Service at 1-800-482-2282 to request a RMA# and issuance of a return freight call-tag. • This service is covered under warranty.
TUSA will service the affected units and return within approximately 7-10 business days after receipt.

Ellipse Black MC5 Regulators Recalled by Cressi USA
Cressi USA has announced a safety recall of its Cressi Ellipse Black MC5 Scuba Regulator. This recall affects Ellipse Black MC5 regulators distributed and sold from March 2009 through August 2009 are affected by this product recall.
The words “Cressi Black” appear on the cover of the regulator’s second stage, and the words “Cressi MC5” are visible on the regulator’s first stage.
Hazard: Partial obstruction of the High Pressure port can produce an inaccurate reading on the pressure gauge, resulting in a slow descent of the needle in the pressure gauge. The inaccurate reading on the gauge poses a drowning hazard to divers.
Remedy: Consumers can return the Cressi Ellipse Black MC5 Regulator to any Cressi-authorized dive shop or directly to Cressi-sub USA at One Charles Street, Westwood, NJ 07675 for free repairs to effected equipment.

NEMO AIR QUICK CONNECTOR RECALL
Dear Customer,
Please be informed that we have discovered a quality issue on the O-ring assembled on the Nemo Air Quick Connector.
Under certain circumstances the O-ring can fail and consequently Nemo Air may start leaking through the Quick Connector. As a result, there is a continuous, albeit slow, loss of breathing gas. This situation could potentially lead to an accident, hence Mares has decided to issue a product recall. The solution already exists in the form of an O-ring of different material and hardness which can be retrofitted onto the Quick Connector of your current Nemo Air.
Please note that the event of an uncontrolled loss of breathing gas from the high pressure hose is regulated by the EN250 Norm, which requires a 0.3mm opening in the high pressure fitting. As a result, the loss of breathing gas is very slow and a diver should always be able to complete a dive safely,

Ambient Pressure Diving Hose Recall
Some hoses fitted to APD products could be unsafe due to insufficiently crimped connector fittings. The fittings should be crimped so that eight flats are clearly visible on the fitting collar. If the collar is smoothly rounded, crimping has not been done. There is a risk of the hose fittings coming away from the hose. Potentially affected are low-pressure hoses used on Inspiration, Evolution and Evolution+ rebreathers; the Buddy Blast Hose (air horn end only); second stage regulator hose (second-stage end only); and gas connection system (second stage/ADV end only). Among the rebreathers, the hoses to examine are the ADV hose (ADV end only); buzzer hose (both ends); oxygen supply hose (solenoid end); and handset hose (lid end). APD suggests that customers who purchased hoses after January 2007 - either as part of another product or as a spare - check their hose-crimping. Apeks Recall of TX/ATX/XTX 2nd Stages
On June 30, 2008, Apeks issued a Consumer Bulletin regarding its TX/ATX/XTX
2nd stage regulators. At the time it was not an official CPSC Recall, and they did not
identify how many regulators had been affected. This consumer bulletin has now been issued as a CPSC recall, so check our "Recalls" page for details. The following is the original warning:
This is limited to those 2nd stages/octopus that have not undergone an
authorized annual service.
It has come to our attention that an incorrectly assembled 2nd
stage has been purchased. If you have an Apeks regulator or octopus that has
never had an authorized annual service, regardless of how recently purchased,
please take the regulator/octopus to your Apeks dealer to be visually inspected.
All Apeks dealers and distributors have been made aware of this
issue. Do not dive with the regulator until it has been visually inspected.
Apeks apologizes for any inconvenience this may cause. We are dedicated to

Recall of Titan/Conshelf DIN Regulators and DIN Adaptors
Aqualung has also posted on its web site a Recall of
Titan/Conshelf DIN Regulators and DIN Adaptors. Again, this is not a CPSC
Recall, and the number of affected units is not known. However, divers using
these regulators are urged to return them for replacement of affected
components, as per the following bulletin:
Aqua Lung was recently informed about the breakage, under
pressure, of aTITAN/CONSHELF DIN HANDWHEEL RETAINER weakened
during service by manual torque loads superior to the recommended values.
Aqua Lung asks all the owners of TITAN DIN regulators whose serial number is
lower than 6062501 or TITAN/CONSHELF DIN adaptors that are
marked 300 BAR MAX to return their regulator to
their Aqua Lung authorized retailer for an upgrade.
The corrective action is replacement of the brass DIN HANDWHEEL RETAINER by
another stainless steel DIN HANDWHEEL RETAINER marked 300 BAR MAXI.
This replacement is made under warranty.
All the TITAN DIN regulators whose serial number are above
6062501 and/or whose DIN handwheel retainer is marked 300
BAR MAXI are assembled with a stainless steel DIN handwheel retainer and
are not affected by this recall.

POSEIDON ISSUES WARNING!
Date: June 16, 2008 Subject: W50 Wings
Poseidon issues the following warning:
There seems to be a manufacturing error on the inner bladder that is
mounted in our W50 wings, which causes the seam that holds the two inner
bladder halves together, to break, making the wing unfit as a floating
device.
It seems to be inner bladders marked with batch number 5445 that are affected by the manufacturing error.
We ask those who have an inner bladder from manufacturing batch 5445 to
contact the dive store where you bought your W50 wing as soon as
possible, so that your W50 bladder can be replaced as soon as possible.
The inner bladders have been mounted in W50 wings sold during the period from September 16, 2007 to June 10, 2008.
To determine if you have an inner bladder affected by this manufacturing error, checkthe following:
- Between the two ´legs´ of the W50 wing, there is a zipper. Open this zipper.
- You should now see the inner bladder. Between the two ´legs´of the inner bladder, there is a tab with the article number,
batch number and manufacturing year.
- If the batch number on this tab reads 5445, the inner bladder might be defective.
- If there is an other batch number, then the bladder is not affected by this warning.
If you have an inner bladder from batch no 5445, we recommend that you
DO NOT dive with the wing, until the inner bladder has been replaced!
Poseidon Diving Systems AB will replace all W50 inner bladders from manufacturing batch no 5445.
Unfortunately, the manufacturing error was not detectable through the
quality control process through which each inner bladder evaluated prior to
delivery.
Poseidon Diving Systems apologizes for the inconvenience this is
causing but we hope to be able to replace these inner bladders as
quickly as possible!

Whites Recalls LP Hose

Hazard: Certain Dry suits that are equipped with inflation valves will also include the low pressure inflator hose. There is a possibility that the low pressure hose quick connect will snap off of the hose. Shipment of hoses in question began on Sept. 1st, 2007.
Incidents/Injuries: None reported.
Dealers will replace the Quick Connect LP Inflate Hose 29”(VLSITINF 32120) with our previous style STD Low Pressure Inflate Hose (29”)(VLSITINF32500).

SeaLife Announces Important Technical Update

Manufacturer: SeaLife
Hazard: SeaLife recommends removing the spiral-sleeve protector or cables
may break.
Incidents/Injuries: Unknown. It has been recognized by our Quality
Control Department that the coiled sleeve which covers the cable inside the
waterproof door can cause stress and possible damage to the cable connection.
Repeated opening and closing of the door may weaken and break this cable. If
the cable does break , the flash will still fire but the flash power adjustment
function would not work.
In order to eliminate this possible breakage, we have removed the wrap from
production and are suggesting that the wrap be removed from any unsold
inventory in your store. Attached you will find a technical update that will
also be available on our website at, www.sealife-cameras.com , that gives simple instructions on
how and what to remove.
Product shipped from SeaLife as of January 21, 2008, will have been updated and
can be identified by a red dot sticker on the outside of the packaging. We will
continue to identify these updated items with these dots until February
15,2008.

Aqua Lung issues the following notice:
Dear Aqua Lung Customer,
It has come to our attention that some S.O.S. units
(PN 394157 and 394057) sold since Sept 2002 may have a faulty component. The
component is known as an "elbow," and is used to transfer air from
the BC to the safety tube. Some elbows have exhibited signs of degradation
leading to cracking and/or breakage. If this were to occur, air may not
successfully transfer from the BC to the safety tube, thus preventing the tube
from inflating. Furthermore, if a diver were inverted, making the attachment
point the "high spot," air could also escape from the BC. The leak may prevent the BC from "fully
inflating." Although the BC will still hold air when vertical, some air
will escape at the bottom and the BC will provide less buoyancy. To date, there
have not been any reported accidents or injuries.
Aqua Lung would like all owners of S.O.S. units to carefully inspect them.
Elbows that have the potential to degrade are easily identified by a molding
gate mark located on the outer bend.
If you have a unit with such a gate mark, we ask that you stop using the
product. Detach it from the BC and return it to your nearest Aqua Lung Dealer.
You may find your nearest Aqua Lung Dealer by using the dealer locator at
www.aqualung.com. Your dealer will return it to Aqua Lung who will replace the
unit at no charge.

There may be a potential problem with the yoke clamp screw on any yoke clamp type regulator with the serial number starting from 7010001 – 7053528. All Apeks dealers have been made aware of this issue.
Please return your Apeks regulator or just the yoke clamp screw to your nearest Apeks dealer for inspection. If you need assistance finding your nearest Apeks authorized dealer, please use the dealer locator found at www.aqualung.com. Do not dive with the regulator until it has been inspected.

PADI Nitrox 32 Tables
PADI Americas is recalling a single version of the
English Imperial EANx32 Recreational Dive Planner used for calculating dive
profiles. A printing error caused a series of numbers to be out of alignment,
thus incorrect. The risk of injury from these errors is remote and no injuries
have been reported.
All RDPs included in this recall bear the printing mark “Printed in the USA”
located on the back of the table, upper right corner. The product number and
production code are located on the lower right corner, also on the back of the
table. The product number and production code affected by this recall are:
Product No. 60049 Ver 1.2 (Rev 02/03) XXXPDK7
The product code digits indicated by “X” may vary. The last four digits “PDK7”
indicate a recalled table, irrespective of what the first three digits are.
As a double check, on Table 3, check the 110 foot row under Pressure Group G.
The table should indicate a Residual Nitrogen Time (upper number in white) of
13 and an Actual Bottom Time (lower number in green) of 12. If this box is
blank, you have a misprinted table.
We will be notifying consumers certified as PADI Enriched Air Divers after 17
January 2008 via the PADI website, e-mail and other means. They will be
directed to stop using the recalled RDP immediately and bring them to any PADI
Dive Centre/Resort or return them to PADI for a free replacement. If you
replace one of these RDPs for a consumer, please notify your Sales Representative
to receive your replacement product.

Zeagle 1st Stage Regulator Recall Dec 2003
Zeagle Systems Inc. is recalling 931 first-stage scuba regulators sold between
January 20, 2003, and May 19, 2003. If you have one of these regulators, he
said, "it is essential to take it to your dealer for immediate repair. Do
not attempt to dive with this regulator again until the regulator has been
inspected and by an authorized Zeagle dealer." The first-stage regulators
affected are the following models: * FlatHead Six, s/n F001391 to F001476; *
50D, s/n B003315 to B003652; * DS-IV, s/n A001352 to A001571; * Envoy, s/n
C001117 to C001403. Serial numbers are stamped on each first-stage body. Zeagle
regulators that have been serviced since January 20, 2003, may also be
affected. These first-stage regulators may have a defective high-pressure seat
that could cause the first stage to over-pressurize. If this occurs, the air
supply to the diver could be interrupted, making it impossible for the diver to
breathe from the regulator. This type of failure is most likely to occur upon
initial pressurization of the regulator and is unlikely to occur underwater.
