Description: This recall involves Halcyon diving equipment including the Halcyon Explorer, Eclipse, CCR35, Evolve and Pioneer Buoyancy Compensator Devices (BCDs) and Halcyon Surface Marker Buoys (SMBs), Lift Bags, Diver Alert Markers (DAMs) Surf Shuttle and Diver Lift Raft Inflatable Devices. “Halcyon” is printed on the diving equipment.

Incidents/Injuries: M&J Engineering has received one report of swivels separating from the masks while diving. No injuries have been reported.
Description: This recall involves all SW-3000 2nd stage 360-degree swivels for scuba diving masks manufactured between January 2004 and February 2005. The swivel has the date stamped on its body, below the patent number. Swivels that are already repaired have an arrow stamped on the top right corner of the swivel body.

Hazard: The swivel, which is attached to a diving regulator, could separate while diving. This will result in a complete and sudden loss of the diver's air supply, causing the diver to engage in emergency ascent procedures. This poses a risk of decompression sickness due to rapid ascent, and air embolism or drowning if the diver panics or the emergency ascent procedure fails. Consumers should stop using recalled products immediately unless otherwise instructed.
Incidents/Injuries: The firm has received one report where the swivel separated during a dive and caused the diver to ascend using the buddy breathing technique. No injury was reported.
Description: The recall involves the HO110 Swivels sold as an aftermarket regulator component. There are no markings such as a date or production code embossed on the unit. Contact Innovative Scuba Concepts for information on where the recalled units were sold.

Subject: Pelagic Recalls Digital Dive Computers Due to Decompression Hazard

Hazard: When performing a switch from one gas to another during a dive, the dive computer's display will lock up and not return to the main dive screen that displays dive times. This can cause divers to enter decompression unknowingly or the diver could ascend prematurely, resulting in decompression sickness.
Incidents/Injuries: Pelagic has received a report of two dive computers malfunctioning. No injuries have been reported.
Description and Models: The recall involves Oceanic-brand ATOM 2.0 dive computers with serial numbers 1 through 2,079 (Revisions 2E, 3A, and 3B) and AERIS-brand EPIC dive computers with serial numbers 1 through 712 (Revision 1A), which can be accessed and viewed on the computer's display. Also, the serial number and date of manufacture are printed on the bottom of the unit (Oceanic ATOM 2.0 from August 23 to November 23, 2006, and AERIS EPIC from October 18 to November 14, 2006). This recall does not include any other Oceanic or AERIS brand dive computers.
Sold at: Authorized Oceanic dealers sold ATOM 2.0 dive computers nationwide from August 2006 through February 2007. Authorized AERIS dealers sold EPIC dive computers nationwide from October 2006 through February 2007. Both computers sold for between $670 and $950.

Description: The D9 and D6 diving instruments are advanced, gas-switching, multi-mode, decompression dive wristop computers. The products included are: the D9 model with serial numbers 62102582 and below; and the D6 model with serial numbers 62103693 and below. The serial number is located on the side of the product. The model number is located on the back of the product.
Sold at: Diving specialty shops nationwide, as well as various Web sites, from September 2004 through June 2006 for about $1,275 for the D9 (without wireless transmitter) and $900 for the D6.

Remedy: Consumers should only use these dive computers with backup instrumentation. Consumers should bring the recalled units to the nearest authorized Suunto dealer for a software update to correct the problem. A free battery replacement and pressure testing will be provided as part of the free software update service. All updated products will be marked with an indelible white dot on the back of the unit, or with an engraved "U" near the serial number.

Hazard: A manufacturing error could cause the main housing of some regulators to change shape over time, which could cause the cover and diaphragm to become dislodged. If this occurs, air flow will be interrupted and the regulators will no longer function, posing a drowning hazard to users.

Description and Models: The recall involves some X650 second stage regulators. All X650 second stage regulators are clearly marked with a blue capital "X" next to the numbers "650" in white. Affected regulators have a visible bump located on the right side of the unit between the letter "C" and where the mouthpiece attaches to the regulator body.

The serial number and "r2A" are printed on the side of the unit. This recall does not include any other Oceanic brand dive computer, or Versa Pro 2A while operated in normal decompression dive computer mode.

Hazard: Over-tightening of the Yoke or Din retainer during annual servicing of these regulators could result in a stress crack and failure of the regulator. If this occurs during a dive, air supply could be interrupted, posing a drowning hazard.
Incidents/Injuries: No injuries or underwater incidents have been reported. Scubapro® USA has received eight reports of units that failed after being serviced.
Description: The recall involves first stage regulators that are marked with the Scubapro brand and identified as the MK20.
Sold at: Authorized Scubapro® dealers nationwide from January 1996 through December 2001 for about $260.

Remedy: Scubapro® USA has been providing a service upgrade kit via the authorized dealer network since May 2005 which contains a Yoke or Din retainer that prohibits over-tightening during annual service. Consumers can easily identify if the service upgrade has been completed by counting the vents on the plastic saddle located on the regulator. If the service upgrade has not been completed, the consumer should bring the regulator to any authorized Scubapro® dealer to have the service upgrade completed free of charge.

Description: The Aqua Scout Water Scooter is a hand-held, battery-powered product used to propel swimmers through the water. The product has a yellow or red plastic enclosure, a black plastic handle and propeller. There is no model numbers or brand name written on the scooter.

Hazard: A slow leak from the shoulder exhaust caused by expansion of an internal cable housing could result in slow deflation. This could impact the diver’s ability to control buoyancy.

Description: Buoyancy Compensator devices provide buoyancy control for scuba divers by allowing them to inflate or deflate the devices. The recalled buoyancy compensators have model numbers J107, J113, J115, and J119. The numbers appear on the top right-hand corner of the unit (on the shoulder). “CRESSI” is written on the buoyancy control devices.
Sold at: Authorized dive shops nationwide and at various Web sites from January 2003 through September 2004 for between $400 and $700.
Manufactured in: Genoa, Italy.
Remedy: Customers can return the product to a Cressi-sub-authorized dive shop to have the product retro-fitted with a free replacement internal cable housing. Buoyancy Compensator devices with a black retaining ring around the yellow inflate button on the inflation mechanism, and an adjoining gray cover assembly (bypass case), which screws onto the inflator, are subject to this recall and must be retro-fitted. Devices which have yellow retaining rings and black cover assemblies have been retro-fitted.

Hazard: The dive computers were improperly calibrated, resulting in incorrect calculations. Diving with an improperly calibrated dive computer can provide inaccurate “No Decompression (no stop) Time,” “Decompression Time,” “Desaturation Time” and “No Fly Time.” This could result in divers suffering serious injuries, including decompression sickness.
Incidents/Injuries: Head USA received one report of a computer malfunction. No injuries were reported.
Description: The recall includes the Dacor Darwin Air dive computer with serial number 3265 through 3415, and the Dacor Darwin Console dive computer with serial numbers 8503 through 8592. The serial number will appear on the dive computer screen after removing and reinserting the batteries. The Darwin Air is a completely integrated dive computer and pressure gauge with “Dacor Darwin Air” written on the front. It is encased in gray on black heavy-duty plastic. The Darwin Console is a dive computer with a compact submersible pressure gauge bearing the word “Dacor” on the front. It is encased in black heavy-duty plastic.

Subject: Ocean Management Systems Recall of SCUBA Manifolds

Name of Product: Rotating Cross Bar SCUBA Manifold

Hazard: Undersized o-rings installed on the cross bar can cause a loss of breathing air from the SCUBA cylinder, presenting a risk of drowning.
Incidents/Injuries: There has been one report of breathing air leaking from a cylinder. No injuries have been reported.
Description: The recall involves the 200 and 300 Bar versions of the OMS Rotating Cross Bar Manifold. The manifold consists of two valves which have “O.M.S.” stamped on the side of each valve and a crossbar that has three O-rings per side. Each part has a black plastic knob.

Hazard: The Power Inflator Buttons can stick, which can cause uncontrolled inflation of the BC. This could cause divers to ascend too fast, resulting in decompression sickness as well as the danger of an embolism.

Incidents/Injuries: Halcyon has reports of 10 incidents of BCs leaking. No injuries or deaths have been reported.
Description: Halcyon is recalling all stainless steel power inflators. They have a stainless steel oral and power inflate button. Inflators made of plastic are not included in this recall. The repaired inflators have a groove at the base of their retainer nut. The BCs sold with these inflators are black and the Halcyon logo is on the front and collar of the BCs.
Sold at: Dive stores nationwide sold these inflators individually from October 2003 through May 2004 for about $46. The inflators also were sold as part of BC units ranging from $270 to $600.

Description: This recall involves Aladin Air X NitrOx dive computers manufactured in 1995. The manufacture date is located on the back of the dive computer, in the lower right hand corner. The date code reads the number of the month followed by a decimal point and the year, for example, "01.95." The dive computers are blue and black with the words "Aladin" and "NitrOx" printed across the front. Aladin Air X NitrOx dive computers with date codes other than "95" are not included in this recall.
Sold At: Scuba diving stores sold the dive computers nationwide from July 1995 through March 1996 for between $950 and $1,200.

The serial number is stamped into the side of the body of the regulator. The regulators have the name "OCEANIC" written on the rubber boot of the first stage. These CDX regulators were sold with the following second stage regulators: Alpha 7, Delta 3, Gamma 2, and Zeta.

The consoles are mostly gray in color and the brand name "UWATEC™" is written on the front of each console. The pressure gauges are made in the U.S. and the computers are made in Switzerland. Description: The recalled regulators have a black body with either a gray, blue, red or yellow faceplate. Printed on the faceplate of the regulator are the words, "Atomic Aquatics"; the model name, "Titanium"; and the model number, T1, T1x, B1 or Z1.
The regulators were manufactured between May 1999 and October 1999, and have serial numbers that range from A07751 to A11276. The serial numbers can be found on a label beneath the rubber mouthpiece.
Sold At: Specialty dive equipment stores nationwide sold the regulators from May 1999 through February 2000 for between $450 and $1,600.

Description: These recalled Durapro Systems BCDs are the Cirrus, Stratus, Aruba, Slimline and Resort. The low-pressure hoses on the BCDs, part number 302, connects the first stage regulator to the BCD power inflator valve. These hoses are identified with the following markings: "PLIOVIC MULTI PURPOSE 1/4" 1/D (6.4 mm) 250 PSI WP (1.7 MPA)...MADE IN USA...GOODYEAR."
Sold At: Retail dive stores and a mail-order catalogue sold these BCDs from January 1996 through May 1998 for about $179 to $359.
Remedy: Consumers who purchased one of thesea Durapro Systems BCDs should stop using it immediately. The hose from the BCD should be returned to the dive store where purchased for a free replacement hose.

Incidents/Injuries: Mares is aware of 45 incidents of the Guardian dive computer failing to operate properly. There have been no injuries or deaths reported involving the use of these dive computers.
Description: The Guardian dive computer comes in two models, a wrist mount and a hose mount. The computer is about 3 inches wide and 3 inches long. It is black and gray with yellow buttons. "Mares" and "Guardian" are written on the frame of the computer around the screen.
Sold At: Selected Mares dive shops sold the Guardian computers nationwide from January through August 1997 for about $400.

Description: The Healthways Ocean Side dive mask is made of fluorescent yellow, marine blue or raspberry plastic and clear tempered glass. The mask was part of a dive set that included a snorkel and fins. The words "Healthways Ocean Side...Tempered" are printed on the front of the mask. The dive set is packaged in a blue, yellow and white cardboard box labeled in part, "Barbados Healthways Ocean Side...4-Piece Dive Set...Made in China for Kmart." The dive set is visible through a plastic window on the front of the box. Sold At: Kmart and Super Kmart stores nationwide sold the dive mask set from February 1997 through August 1997 for about $20.

Incidents/Injuries: Scubapro has received six reports of the VIVA control knob detaching from the regulator, two of which occurred during a dive. No injuries have been reported.
Description: The G500 second stage regulators involved in this recall have serial numbers less than or equal to 13497450. The serial number is located on the bottom face of the exhaust manifold.
Sold At: These products were sold in diving specialty stores nationwide from March 1997 through June 1997 for around $300.
Remedy: Consumers should immediately stop using G500 second stage regulators involved in this recall and contact their local, authorized Scubapro dealer to arrange for a free replacement of the VIVA control knob. Under no circumstances should consumers dive with these regulators until they have been repaired.

Remedy: If consumers notice the battery has flooded during a dive, they should disassemble the battery's retaining ring and bulkhead to prevent a possible fire. Consumers should stop using the Source-12 Battery Packs immediately and call LMI for free repair or upgrade information. Subject: Trident Diving Equipment Recalls Scuba Diving Tank Valve Caps
Product Name: Screw-in type DIN Tank Valve Caps

Hazard: If tank pressure is 3000 psi or greater, the valve cap is not properly inserted, and the air valve is on, the valve caps may fracture and pop out of the air tank, causing injury.
Incidents/Injuries: Trident is aware of one incident of injury where a valve cap allegedly popped off a tank and struck the consumer in the hand.
Description: The valve cap, Trident Model RP73, is intended for use on SCUBA diving compressed air tanks to prevent sand, dirt or dust from contaminating the area around the valve. The black plastic caps measure 1" in diameter and 1" thick. The hexagon-shaped caps can be tethered to tanks by a string attached to a plastic eyelet in the top of the cap.
Sold At: The valve caps were sold between January and June 1995 through Trident's catalog and at dive shops nationwide. The caps cost between $2.00 and $4.00 and were packaged in unlabeled plastic bags.

Subject: DUI Recalls Dry Suit AC Valve

Hazard: The valve may free-flow causing air to fill the dry suit resulting in the diver rising rapidly to the surface or failing to provide sufficient air to maintain buoyancy. The possibility exists of serious injury and/or death, especially if appropriate emergency procedures are not followed.
Incidents’Injuries: The Consumer Product Safety Commission is aware of incidents involving 16 divers which are associated with AC valve malfunction.
Description: The DUI AC valve is used on SCUBA diving dry suits. The DUI AC valve deflates or inflates the diving dry suit to allow the diver to equalize air pressure while ascending or descending in the water by pressing on the valve. The AC valve differs from the standard 2-valve system commonly used in dry suits, where air intake and outflow are controlled by two separate valves.
Remedy: Owners of dry suits with AC valves attached should immediately stop using the dry suits. DUI asks that all divers return their suits (at DUI's expense) and have the AC valve replaced at no charge. Divers who have one of these AC valves attached to their dry suits should contact DUI on their toll free number 800-327- 8439 to obtain valve replacement instructions and a FREE dry suit check-up. All dry suits returned to DUI will be tested and DUI will fix any leaks in the suit (except for seals) at no additional charge. Further, if the diver wishes to upgrade the valve system, this will be done at the manufacturer's cost of the valves only.
